A scanned copy or a photo of the applicant's identity card (ID card or passport).

Notice: The submission of a forged or altered identity document may fulfil the elements of the criminal offence of fraud pursuant to Section 209 and the criminal offence of forgery and alteration of a public document pursuant to Section 348 of Act No. 40/2009 Coll., the Criminal Code. Such cases are referred to the Police of the Czech Republic for review.
